The TMC Hospice staff is specially trained to help you remain as pain-free and alert as possible so that your last days may be spent with dignity and confidence in your own home, surrounded by those you love. Care is provided by a team of professionals and volunteers.
Medical DirectorThis licensed physician has special skills in caring for people with terminal illnesses and ensures that each patient receives the medically indicated services needed to control the symptoms of the patient's terminal illness. At TMC Hospice, our medical director is a valuable resource to the patient, the family and hospice staff in managing the symptoms that arise from time to time. In addition, consultation with the TMC Hospice medical director is available to your physician.Attending PhysicianAn attending physician is the medical doctor who oversees all of a patient's care. At TMC Hospice, our medical director often serves as a patient's attending physician in close consultation with the patient's primary care physician. A patient's attending physician is more than welcome to continue following their patients' care while they are on hospice.Primary Registered NursePatients and their families are assigned a primary nurse who works directly with the patient's personal physician to provide care that includes:
- Prevention and control of symptoms and pain
- Coordination of care with the patient's physician and the TMC Hospice medical director
- Education and guidance for families who are caring for their loved ones
- Teaching of daily care routines to the family
- Arranging for medical equipment and supplies
- Assists patients with personal care (bathing, hair care, shaving, changing of linens, etc.)
- Teaches personal care routines to patients and their families under the supervision of a registered, professional nurse.
- Assists patients in transferring in and out of bed and to exercise.
- Offer companionship and support to patients and family members.
- Stay in the home for a limited time so family members can shop or take care of personal business.
- Shop, run errands and transport patient and family to appointments.
- Provide light housekeeping to ensure a safe living environment.
- Help with urgent needs when necessary.
- Provides spiritual support and guidance when requested.
- Coordinates spiritual care with local religious leaders.
- Helps patient and family deal with feelings of anger, frustration and grief.
- Facilitates regular support groups and provides individual counseling to adults and children.
- Provides special memorial services for families, staff and volunteers.
- Refers families to specialized services in the community as needed.
- Assessment of the social and emotional factors related to the patient's illness, the patient's need for care, the patient's response to treatment and the patient's adjustment to care.
- Appropriate action to obtain case work services to assist in resolving requirements to the patient's home situation, the patient's financial resources and the community resources available to the patient.
